Administrative and clinical information for all NHS Wales Accident and Emergency department attendances. Includes the All Wales Injury Surveillance Systems (AWISS) dataset.

Data recording practices may vary, especially in some of the minor A&amp;amp;amp;E and Minor Injury Units which could account for some local differences.

This dataset requires additional governance approvals from the data provider before data can be provisioned to a SAIL project.

DHCW

Site Code of Treatment (5 character code). The organisation code for the site where the patient will be or is treated.

SITE_CD_OF_TREAT

All data related to the Emergancy Department Data Set:
The Emergency Department Data Set (EDDS) captures attendances at Accident &amp;amp;amp; Emergency departments and Minor Injury Units (MIUs) in Welsh hospitals.
Velindre NHS Trust does not provide EDDS data as there are no A&amp;amp;amp;E departments or MIUs in the Trust.
Welsh residents who attended an emergency department in England are not included.
The submission is based on administrative end date.
Planned follow up patients are included.
Definitions that apply to SITREPS may differ from those associated to the EDDS.

EDDS

Provider Unit Code (3 character code). This is the organisation code of the health care provider. The provider code identifies the health care provider who is responsible for managing the treatment of the patient.

Notes:

1.  Healthcare providers may also act as commissioners when sub-contracting patient care services to other providers of health care.

2.  Although the healthcare provider identified in this data item is responsible for managing the patient&amp;amp;acirc;&amp;amp;euro;&amp;amp;trade;s treatment, it may not necessarily be where the treatment is actually conducted. For example, where the treatment has been sub-contracted to another healthcare provider.

3.  For OPRD (outpatient referral dataset), the Organisation Code (Code of Provider) is that of the organisation receiving the referral. If the provider is a Local Health Board/Trust, use the 3 character Local Health Board/Trust code with 2 zeros placed in the 4th and 5th character position.

4.  For RTTD (Referral to Treatment Times (Combined)), use the 3 character Local Health Board/Trust code.

PROV_UNIT_CD

A general reason for an Accident and Emergency Department Attendance.

Note: &amp;amp;acirc;&amp;amp;euro;&amp;amp;tilde;30 &amp;amp;acirc;&amp;amp;euro;&amp;amp;ldquo; Dead on Arrival&amp;amp;acirc;&amp;amp;euro;&amp;amp;trade; should only be recorded for patients who have been certified as dead on arrival by a clinician in the Accident and Emergency Department

ATTEND_GROUP

Sixth diagnosis type code, which may have beeen identified during an Accident and Emergency Department Attendance.

Notes:
- At least one Diagnosis Type must be reported
- Up to 6 diagnosis types can be reported in this section of the Data Set

If the referral is not from a GMP, GDP, Consultant or Independent Nurse one of the default codes used.

First indication of the type of treatment or guidance which may be provided to a patient as a result of an Accident and Emergency Attendance.

This is the field to identify the type of record being submitted.

RECORD_ID

Fifth indication of the type of treatment or guidance which may be provided to a patient as a result of an Accident and Emergency Attendance.

TREAT_CD_5

Accident and Emergency Attendance Category is an indication of whether a patient is making a first or follow-up attendance at the Accident and Emergency Department.

Note - possible values
- New Attendance. A new attendance is the first visit made by a patient to the Accident and Emergency Department for a particular injury or ailment. A new attendance can be the first in a series or one single attendance in an Accident and Emergency Department.
- Planned Follow-up Attendance. A follow-up Accident and Emergency Attendance is a subsequent planned attendance at the same department, and for the same incident as the first attendance. A planned follow-up attendance is agreed or requested by a clinician.
- Unplanned Follow-up Attendance. A patient returning to the Accident and Emergency Department with a conditional previously treated where they have not been asked to return by the clinician will be counted as an unplanned follow-up attendance.

ATTEND_CATEGORY

Match percentage assigned when deriving the Encrypted Anonymised Linking Field.

ALF_MTCH_PCT

Alcohol Indicator. In the clinical opinion of the Emergency Department, has the presence of alcohol in the presenting patient contributed to this attendance?

Note: this question only needs to be answered for New Attendances and Unplanned Follow-ups. For Follow-up patients, value &amp;amp;acirc;&amp;amp;euro;&amp;amp;tilde;04 &amp;amp;acirc;&amp;amp;euro;&amp;amp;ldquo; Not Applicable&amp;amp;acirc;&amp;amp;euro;&amp;amp;trade; should be recorded in this field.

ALCOHOL_IND

Additional Incident details. A record of any additional details of the incident that led to the Accident &amp;amp;amp; Emergency Department Attendance that cannot be recorded elsewhere within the dataset.

Note: This field allows for any supplementary information supporting either the injury or illness causing the patient to attend the Accident &amp;amp;amp; Emergency Department, or any other details pertaining to the patient&amp;amp;#039;s treatment, to be recorded; this includes details relating to the location where the incident occurred specifically, Road, Vehicle collision or Assault incidents.

ADD_DETAILS

Lower Layer Super Output Area (LSOA) code in which the patient is resident, for a previous version of LSOA codes (2001)

The encrypted GP practice code where the appointment occured.

PRAC_CD_E

The sport in which the patient was participating at the time of injury that led to the Accident and Emergency Department attendance.

This should only be completed when the &amp;amp;#039;Activity at Time of Injury&amp;amp;#039; is &amp;amp;#039;03 &amp;amp;acirc;&amp;amp;euro;&amp;amp;ldquo; Sport&amp;amp;#039;

SPORT

Encrypted Ambulance Incident Number. When a patient arrives by ambulance, this is the incident number for that journey, allocated by the Ambulance Control.

For patients who did not arrive by ambulance, this field should be left blank.

AMB_INCID_NO_E

This is the sex (gender) of person, employee or patient.

TREAT_CD_6

The status indicator provides information about the potential accuracy and reliability of the NHS number and hence the use to which the number can be put.

The indicator can also be used to indicate the general standard of patient data quality within Trusts.

This data item became mandatory in Wales in April 1999.

NHS_NO_IND

Administrative End Date. This is the date that the patients Accident and Emergency Attendance Ends. 

Accident and Emergency Attendance Administrative End Date is when one of the following criteria is met;
-  The patient leaves the Accident and Emergency Department to continue their treatment elsewhere, wherever this might be, this is the point of clinical handover;
-  The patient is discharged and is medically fit to leave the department;
-  The patient self discharges;
-  The patient dies.

ADMIN_END_DT

ROAD_USER

The Anonymised Linking Field, which has been Encrypted for its use within the database, is derived from the persons NHS number by double encryption (first encryption occurs in NWIS and the second in SAIL). If the NHS number is not supplied in the data extract then matching methods are applied.

ALF_E

The type of place where the physical or chemical injury occurred which led to the Accident and Emergency Department Attendance.

LOCATION_TYPE

Administrative End Time. This is the time that the patients Accident and Emergency Attendance Ends.

Administrative End Time is when one of the following criteria is met;
